Transitional care is a benefit offered to Medicare patients after they have been discharged from a hospital, skilled nursing facility, long term care, rehabilitation center, or observation unit.
After you are discharged from a facility your focus should be a road to healing and recovery. Here at Transitional Healthcare of America we take the burden of transitioning from facility to home for you. Our team will work with you to arrange appointments, prescriptions and more to allow you to focus on your recovery. If you already have a primary provider or practitioner no problem, we arrange with your healthcare team your appointments so that you can be seen as soon as possible.
If you do not have a primary care doctor, we will help you find one. At Transitional Healthcare of America allow us to take care of you so you can focus on getting better.
